They poll people who follow highschool basketball throughout the country, and base their rankings on their opinions. ESPN might not know who the best players in Nebraska are, but they have a correspondent or two or three out that way who keep their eye on basketball in that state and are paid to report back. Most of those correspondents also earn money by alerting colleges they're affiliated with about players who have college or pro potential.
High school coaches know how to contact those guys. If a HS coach has a great player on his team, he'll let that scout know about it, just to promote his own agenda.
Yes, it's possible that there is some awesome high school player out there that ESPN missed, but it's not likely.
